所有董事需提供以下文件All directors should provide:
1. 至少2个证件过KYC系统 At least 2 Photo IDs to pass KYC system:
- Passport 护照
- Australian Driver's Licence (both sides) 澳洲驾照正反面
- Medicare (both sides) 医保卡正反面
- National Identification Card (both sides) 身份证正反面
2. 证件+地址证明（3选1）Photo ID + Proof of Address (choose one of three)
- Passport护照 + Australian Driver's Licence澳洲驾照
- Australian Driver's Licence澳洲驾照 + Medicare医保卡
- Passport护照 + Proof of Address（Issued within 3 months）近三个月内生成的地址证明
3. 手持护照或澳洲驾照自拍Handhold Passport or Australian Driver's License selfie

所有持有公司股份25%及以上的最终受益人需提供：All UBO who holds 25% or more shares of the company should provide：
1. 至少2个证件过KYC系统 At least 2 Photo IDs to pass KYC system:
- Passport 护照
- Australian Driver's Licence (both sides) 澳洲驾照正反面
- Medicare (both sides) 医保卡正反面l National Identification Card (both sides) 身份证正反面 
2. 近三个月内生成的地址证明Proof of Address (Issued within 3 months)   

所有董事都需要在表格上签名, 签名需要与“证件上的签字”匹配

All directors need to sign the form. Signature needs to be matched with Signature on ID
